Fit Health Care Telehealth Instructions

In order to comply with Governor Ducey’s Executive Orders pertaining to social distancing amid the COVID-19 pandemic, Fit Health Care has decided to move forward with providing Telehealth appointments. We want to ensure you that you will continue to experience the same exceptional care that you would with an in-office appointment.

On the day of your appointment 15 minutes prior to your appointment time, please call the office at (602-279-5049) to ensure that we have current contact information and pharmacy information.

If you have a copay or visit charge it will be collected during the call. If there are attachments to this email please complete forms and return them to the office prior to your appointment.

If you need to cancel or reschedule your Telehealth appointment, please call the office and give as much notice as possible. As with all appointments , you will be charged a $25 missed appointment fee if you do not cancel your appointment AT LEAST 24 hours’ notice.

After your copay is made, you will be able to log into your Telehealth appointment. You may use the following browsers: Google Chrome, Firefox, or Safari. In addition, most smart phone bowsers may be compatible. If you are using your smartphone for the session, PLEASE turn off and restart it before session. Double check your device and ensure that your browsers are running with the latest versions. Please do not use Internet Explorer or other Microsoft browsers, as they are not compatible with Doxy.me technology.

Welcome To FIT Health Care Clinic!

Are you looking for a Primary Care Provider (PCP)? FIT offers a full range of Family Practice Services and can be your medical home.

Our Phoenix “Family” Practice clinic is open 5 days a week for your convenience. (we do close many major holidays-call for information)

Sexually Transmitted Disease (STD) testing walk-ins are welcome from 9:00am to 4:30pm Monday – Thursdays and 9:00 am – 1:00pm on Fridays. Fit Healthcare Clinic closes from 1-2pm daily for lunch. Please contact us for holiday testing hours schedule.

Scheduled appointments for medical care are also available during normal business hours. Please Call 602-279-5049 to schedule and if you have any questions or need clarification.

FIT office visits start at $65. For the non-insured, underinsured or those not wanting to use their insurance. We also offer lower cost “cash-pay” labs for those without insurance or who don’t want to use insurance because of high deductibles.

FIT takes many commercial, Medicaid/AHCCCS, Medicare and Market place Insurance plans- please call for information- (PLEASE have your insurance card with ID number and group number available when you call)

In partnership with the public health department, we are excited to offer the following service:

“ASK FOR “THE CORE 4 COUNTY TEST”

$90 for an STD evaluation & testing for Chlamydia, Gonorrhea, HIV and Syphilis. Including medications for the treatment of Gonorrhea, Chlamydia and Syphilis (as needed). This is a test that involves a simple blood draw and urine sample. Results are back in 3 to 7 days.

Been coming here for 4 years, since 2015. As long as you call a week ahead of when you need in, its normally no issue to get an appointment. And they offer many walk in services as well. Once you come in for your appointment, or even a walk in service, the wait times are always very reasonable. I’m normally in and out within 30-40 minutes. I always leave happy, as the doctor and staff are very caring and friendly, and again, the visits hardly takes up any of my time which is a huge plus!

I recently made another visit to check out their new office (as well as get a check up) and am even happier with the new location. Cameron is an awesome addition to the office. He speaks to you like you are a person and just having a casual conversation, yet stresses the importance of medical treatments.
